SCForm is a simple contact form written in PHP.

Yeah, there are a lot of "contact form" type scripts out there. But after looking at all I could find, and even installing and testing a few, I found that none of them offered just the mix of simplicity and functionality I desired. Basically, I wanted something that was both fairly simple and that was secure (including the hiding of recipients' email addresses!). So I wrote my own. Yes, I'm afraid it's: Yet Another Contact Form ;).

Important: Each package you find at JIMSUN has been signed with a PGP signature. You are urged to validate any package you download by checking it against its signature. You will need Jim's PGP Public Key

SCForm: Simple Contact Form

SCForm is designed to be simple, yet flexible and secure. It features: Check the README (links below, where the download links are) for more detail.

SCForm was originally written and tested in the following environment:

NOTE: SCForm uses PHP's mail() function. Thus it requires there be working mail server software on the host upon which SCForm is installed and that php.ini be properly configured to use it.

Source & Docs: Production Version

Production versions have been tested more thoroughly, at more sites.
If versions 1.2.0 and below did not work for you on a Microsoft Windows server, and you've carefully reviewed both the README and the FAQ, please try version 1.2.1 or greater. Please let me know if doing that worked. Thanks.

Source & Docs: Beta-Test Version

Beta versions are the result of enhancement requests and bug reports. While also believed to produce correct results (maybe even more accurate or better results--depending on the reason for the change), they're labeled beta until I get enough feedback to let me know all's well. (Or I fail to get any negative feedback in the form of bug-reports.)
If versions 1.2.0 and below did not work for you on a Microsoft Windows server, and you've carefully reviewed both the README and the FAQ, please try version 1.2.1 or greater. Please let me know if doing that worked. Thanks.
